Please submit this form below as Verification of Ownership. It is not necessary to submit the white registration card that came with your rangehood, if you are using this form. You can find the model and serial number of your hood by looking for the white UL label. It is located on the inside of the hood underneath the grease filters. Be sure your rangehood is turned off before locating the Model & Serial Number. Please retain a copy of your sales invoice as additional verification.
